We are currently seeking a dedicated and compassionate Home Care Nurse to join our team. The Home Care Nurse will be responsible for providing nursing care to patients in their homes, assisting with activities of daily living, administering medications, monitoring vital signs, and collaborating with other healthcare professionals to ensure the best possible outcomes for our clients.

**Responsibilities:**
- Assessing patients'' health and needs in the home setting
- Developing and implementing individualized care plans
- Administering medications and treatments as prescribed
- Monitoring and recording patients'' vital signs
- Educating patients and their families on managing illnesses and injuries
- Collaborating with physicians and other healthcare professionals
- Ensuring the safety and well-being of patients at all times

**Qualifications:**
- Valid nursing license or certificate
- Proven experience as a Registered Nurse or Licensed Practical Nurse
- Excellent communication and interpersonal skills
- Compassionate and caring attitude
- Ability to work independently and as part of a team
- CPR certification is preferred
